LabCorp: Patient Service Center Locator
Dear Client: You must have a multi-part Lab Corp chain of custody and control form as well as a picture I.D. to bring to LabCorp for your drug screen collection.

Enter City and State OR zip code.
You MUST select Drugs of Abuse to find a LabCorp facility to collect your drug screen.
Please be sure to confirm the HOURS for Toxicology (drug screen) collections.
Quest Diagnostics: Patient Service Centers
Dear Client:
You must have a multi-part Quest chain of custody form as well as a picture I.D. to bring to Quest for your drug screen collection.
Please select "Drug Screen Collection" and put in your zip code and/or City and State.
You may use any QUEST DIAGNOSTICS location for your agreed upon price. If you use a "3rd party preferred site" (which is a doctor's office, not Quest), you will be billed an additional $15 for that collection.
